TASC seeks a Care Coordinator in Chicago to support the CRT division, helping clients navigate discharge planning and link them to community-based health services. The role emphasizes health equity and collaboration with partners to improve outcomes for justice-impacted populations.

The position offers a hybrid work schedule with three days on-site and up to two remote days per week, starting salary $44,676–$47,000, contingent on experience and education, with a strong focus on preventive care

Qualifications

• High school diploma or GED; valid driver's license.
• CADC certification within two years may be required.
• Must pass an IDOC Background check.
• Proficient in Microsoft Office suite.
• Strong group facilitation and interpersonal skills.

Responsibilities

• Coordinate discharge planning and linkage to long-term providers.
• Educate patients on primary care and preventive services.
• Provide care coordination and navigation for health resources.
• Maintain accurate program documentation and reports.
