TASC (Treatment Alternatives for Stronger Communities) in Chicago is seeking a full-time Recovery Liaison to provide specialized case management for adults with behavioral health needs, referred from Juvenile Court. Serve as initial contact post-assessment and arrange first treatment referrals based on individual needs.

Ideal candidate has a Bachelor’s degree (preferred) and may pursue CADC/CARS within two years; must pass a DCFS background check and have a valid driver’s license.

Qualifications

• Bachelor’s degree preferred and/or three years’ lived or professional experience.
• Ability to obtain CADC/CARS certification within the first two years.
• Must pass DCFS background check and have a valid driver’s license.

Responsabilités

• Provide direct case management and coordinate services with behavioral health and public agencies.
• Make contact with the referred client within 48 hours of notification and provide mobile assessment services.
• Contact parents to facilitate JCAP assessments and re-engage individuals into SUD treatment.
• Complete intake packets for enrollment and participate in court proceedings when needed.
• Document screenings, enrollments, service plans, progress notes, and discharge summaries.
