Treatment Alternatives for Stronger Communities (TASC) seeks a full-time Care Coordinator to support the CRT division’s mission of connecting individuals with substance use and mental health services. The role emphasizes coordination with hospitals, primary care, and community providers to ensure timely access to preventive and specialty care and to support recovery-oriented planning.

Qualifications

• High school diploma or GED required.
• Valid driver’s license required.
• CADC certification within two years may be required based on contractual obligations.
• Must pass an IDOC background check.
• Proficiency in the Microsoft Office suite.

Responsibilities

• Lead initiatives to improve healthcare outcomes and reduce health disparities for justice-impacted populations.
• Enhance care integration, efficiency, and coordination across provider types and care levels.
• Coordinate and implement delivery of consistent, quality program services.
• Prepare and present reports to supervisors and partners within established timeframes.
• Identify and refer individuals to social services addressing social determinants of health.
