Job summary

Professional Services Group & Community Impact Programs (PSG/CIP) is seeking a compassionate Care Coordinator to support adults in the CCS program in Milwaukee County. You will deliver wraparound, recovery-based services in home and community settings, focusing on strengths and person-centered care.

Responsibilities include assessments, recovery plans, team coordination, crisis intervention, and advocacy. Hybrid/remote scheduling and local travel are part of the role; a Bachelor's degree is

Qualifications

• Bachelor's Degree in Social Work, Psychology, Criminal Justice, or related field (required).
• Valid driver's license, reliable daily transportation, and insurance.
• 1+ year experience working with individuals with mental health or substance use needs (preferred).

Responsibilities

• Conduct comprehensive strengths and needs assessments during intake and throughout services.
• Develop and implement individualized Recovery Plans of Care.
• Coordinate and lead team meetings with service providers and support networks.
• Provide crisis intervention and safety planning as needed.
• Attend court hearings, psychiatry appointments, and provide advocacy support.
• Safely transport clients to appointments when necessary.
• Build strong formal and informal support teams.
