Job summary

Thresholds in Chicago seeks a Service Coordinator to apply clinical skills to aid mental health recovery. You will coordinate services, build rapport with clients, and connect them with medical, housing, and other resources to enable safe, healthy living.

The role emphasizes harm reduction, trauma-informed care, and housing first models. You will work on a multidisciplinary team, meet clients in various settings, and advance professional development through supervision and training opportunities

Qualifications

• Bachelor’s degree in social work, psychology or closely related field required.
• Master's degree in social work, counseling or closely related field preferred.

Responsibilities

• Supports the mental and physical health care of members on an assigned caseload.
• Coordinates care with a member’s mental health, physical health, and/or substance use providers.
• Works with community resources to coordinate member services.
• Provides support to remove obstacles to outpatient care and coordinates referrals.
• Facilitates outreach, engagement, linkage, referrals, and follow up for services.
• Identifies gaps in care and arranges timely resolution with agencies.
• Develops relapse prevention plans with clinical consultation.
• Advocates for vulnerable members.
• Provides education on mental health, physical health, and substance use disorders.
• Delivers brief interventions using CBT or MI as indicated.
• Participates in caseload consultations.
• Ensures grant compliance and connects participants to employment services and benefits.
• Ensures compliance with service delivery, data quality, and outcomes for permanent supportive housing programs.
• Collaborates with Housing Administration, HMIS data admin and Grants Accounting on reports.
• Completes eligibility and needs assessments from CES referrals.
• Monitors service delivery of project partners; reviews activity reports.
• Conducts annual audits and file reviews for providers.
• Keeps supervisor informed on project operations and wellbeing.
• Other duties as assigned.
