Humana Inc. is seeking a Field Care Manager, Behavioral Health 2 to assess member needs in home or community settings and guide families toward appropriate resources. The role requires coordinating with the ICT, developing care plans, and addressing barriers for optimal outcomes.

You will travel regionally (15–20%), work a hybrid schedule (80% remote / 20% field), and support adults with behavioral health needs. The position includes a 40-hour workweek and competitive benefits.

Qualifikationen

• Must reside in the State of Michigan with the ability to travel to Wayne and Macomb counties (15–20%).
• Bachelor’s degree in Social Work (BSW) from an accredited institution.
• Active Michigan social work licensure in good standing (LLBSW, LBSW, LLMSW, LMSW, or LCSW).
• 2+ years of post-degree clinical or care management experience in behavioral health, medical, or community-based settings.
• Experience with adult populations, chronic conditions, and disease management.
• Knowledge of community health resources and support programs.
• Ability to travel for home visits and in-clinic assessments; valid driver’s license.

Aufgaben

• Manage a caseload and complete comprehensive assessments with members in their home or community-based settings, and telephonically.
• Provide clinical support and guidance with the Interdisciplinary Care Team (ICT) to support members with medical complexity.
• Develop and coordinate individualized care plans ensuring appropriate services are received.
• Identify and address barriers to care while advocating for optimal member outcomes.
• Review and complete medical attestations and provide ongoing clinical oversight.
• Ensure services are delivered in the least restrictive setting through ongoing assessment.
• Modify Individual Care Plans with care team members and informal caregivers as needed.
• Support members and caregivers using an interdisciplinary approach to access social, housing, and community resources.
