Job summary

Qualifications

• Must reside in Michigan with ability to travel to Wayne and Macomb counties (15–20%).
• Bachelor’s degree in Social Work (BSW) from an accredited institution.
• Active Michigan social work licensure in good standing, including LLBSW/LBSW/LLMSW/LMSW/LCSW. Limited license holders must show progress toward full licensure.
• 2+ years of post-degree clinical or care management experience in behavioral health, medical, or community settings.
• Experience with adult populations, chronic conditions, and disease management.

Responsibilities

• Manage a caseload and complete comprehensive assessments in home, community, or telephonic settings.
• Provide clinical support and guidance coordinating with ICT to support medically complex members.
• Develop and coordinate individualized care plans to ensure appropriate services.
• Identify barriers to care and advocate for optimal outcomes.
• Review medical attestations and provide ongoing clinical oversight.
• Ensure services are in the least restrictive setting to maintain well-being.
• Develop and modify Individual Care Plans with care team involvement.
• Assist members/caregivers in accessing social, housing, educational, and community resources.
