Humana is seeking a Field Care Manager, Behavioral Health to assess member needs and guide families toward resources that support wellness. The role requires independent judgment to determine appropriate actions and may include creating member care plans.

Responsibilities include evaluating physical, environmental and psychosocial health, identifying barriers to care, and monitoring progress toward outcomes.

Qualifications

• LCSW, LPC, LAC, or LMFT licensure required or applicable in the state.
• Experience working with Department of Children and Family Services preferred.
• Managed Care experience preferred.

Responsabilités

• Assess and evaluate member needs to achieve optimal wellness and guide families toward appropriate resources.
• Monitor patient care through assessments and evaluations to ensure progress toward outcomes.
• Create member care plans and identify/resolve barriers to care.
• Occasional travel to Humana offices for training or meetings may be required.
