Humana is seeking a Field Care Manager, Behavioral Health II to assess member needs and guide families toward appropriate resources. The role involves home visits, telephonic coordination, and collaboration with the Interdisciplinary Care Team to support medically complex members.

The position requires Michigan residency, licensure, and 2+ years of relevant experience, with 15–20% travel. Hybrid work arrangement in Michigan fields and community settings supported by robust EMR tools.

Qualifications

• Must reside in Michigan and be able to travel 15–20% to Wayne & Macomb counties.
• 2+ years of post‑degree clinical or care management experience in behavioral health, medical, or community settings.
• Experience with adult populations, chronic conditions, and disease management.
• Knowledge of community health resources and social service programs.
• Ability to travel to facilities and conduct in‑home assessments, including telephonic engagement.

Responsabilités

• Manage caseload and conduct comprehensive assessments in home, community-based settings, or by phone.
• Provide clinical support and guidance within the Interdisciplinary Care Team.
• Develop and coordinate individualized care plans to ensure appropriate services.
• Identify barriers to care and advocate for optimal outcomes.
• Review medical attestations and provide ongoing clinical oversight.
