Qualifications

• Ability to travel 15–20% within Wayne and Macomb counties.
• Valid driver’s license and reliable transportation.
• Experience with adult populations and chronic conditions.

Responsibilities

• Manage a caseload and complete home or community assessments.
• Provide clinical support and coordinate with the Interdisciplinary Care Team.
• Develop, modify, and coordinate individualized care plans.
• Identify barriers to care and advocate for member outcomes.
• Oversee medical attestations and clinical supervision.
• Ensure services in least restrictive settings and monitor needs.
• Collaborate with care team and informal caregivers to access resources.
• Serve as primary ICT point of contact for care delivery.
