Job summary

Humana is seeking a Field Care Manager, Behavioral Health to assess and coordinate care for members, guiding families toward resources and planning for optimal wellness. The role involves independent judgment in ambiguous situations and adherence to guidelines.

You will work remotely with occasional travel to Humana offices for training, with 40 weekly hours and a base salary range of $65,000–$88,600 plus bonus eligibility. LCSW/LPC/LAC/LMFT licenses and DCFS experience are preferred.

Qualifications

• LCSW, LPC, LAC or LMFT license in good standing.
• Experience with Department of Children and Family Services.
• Applicable state licensure in the field of study.
• Passion for improving consumer experiences.

Responsibilities

• Assess member needs and guide toward appropriate resources.
• Monitor care and assess progress; create member care plans.
• Collaborate with care teams and follow guidelines and procedures.
