Job summary

Humana is seeking a Field Care Manager, Behavioral Health to assess and coordinate care for members across home settings. You will evaluate needs, develop care plans, and guide families toward services while collaborating with clinicians and community resources.

This remote role requires licensure (LCSW/LPC/LAC/LMFT), experience with DCFS, and the ability to work independently while following Humana guidelines. Expect travel to Humana offices for training as needed.

Qualifications

• Must hold licensure in field of study (LCSW/LPC/LAC/LMFT).
• Experience working with Department of Children and Family Services (DCFS).
• Applicable state licensure in the field of study.

Responsibilities

• Assess and evaluate members' needs to achieve and maintain optimal wellness.
• Guide members and families toward appropriate care resources and facilitate interaction with care teams.
• Identify and resolve barriers to care and monitor progress toward outcomes.
