Qualifications

• Bachelor's degree in social work or related field preferred; CHW certification within two years.
• 2+ years of experience in social service delivery with elderly and family population.
• Demonstrated working knowledge of supportive services and resources in the served area.
• Ability to advocate, organize, problem-solve, and deliver results for residents.

Responsibilities

• Resident Documentation and Recordkeeping in Apricot or designated systems per requirements.
• Program Enrollment and Documentation for residents in the Service Coordination program.
• Resident Assessment and Care Planning to identify needs and goals.
• Referral Coordination and Follow-Up with community health resources.
• Quarterly Monitoring of care plans, progress and services.
• HUD Reporting and Grant Compliance documentation for funding.
• Quality Assurance and File Compliance per QA standards.
• Resident Programming and Wellness support.
• Program Standards and Performance alignment with mission.
• Training and Professional Development for staff.
