Job summary

Thrive Alliance in Indiana seeks a Community Health Worker to join a care team, providing education, advocacy, care coordination, and practical support to individuals across homes and community sites.

You will work with seniors, disabled, and other clients, documenting services in data systems and coordinating with healthcare providers to improve outcomes and independence. This is a full-time role located in Decatur County, field-based with home visits and community settings.

Qualifications

• High school diploma or equivalent required.
• CHW certification required, or complete approved CHW training and obtain certification within six months.
• Two years of relevant experience in community health, healthcare, care management, human services, a community-based organization, or a Medicaid/Medicare environment is preferred.
• An associate or bachelor's degree in human services, psychology, nursing, public health, social work, or a related field is preferred.
• Experience working with older adults, individuals with disabilities, vulnerable populations, healthcare providers, state agencies, or Medicaid/Medicare processes is preferred.

Responsibilities

• Services are delivered in homes, community settings, by telephone, or at program locations and documented in required data systems.

About the Organization Thrive Alliance helps individuals access resources and support services that improve their lives and builds stronger communities. As the "umbrella" over several programs, we provides services that help the elderly, disabled, and low-income individuals in 5 counties of south central Indiana: Bartholomew, Brown, Decatur, Jackson, and Jennings. These programs include:
Aging and In-Home Services - as the local Area Agency on Aging, Area 11, Thrive Alliance provides Care management and resources to assist those over the age of 60 to remain at home or in a community setting.
Aging and Disability Resource Center (ADRC): a highly visible and trusted place where people can turn for information on a full range of support options.
Adult Guardian Services: Provides guardianship services for disabled adults in need of support and legal assistance.
Nutrition Services: Provides congregate meal site where seniors can come have a hot meal and interact with others in a comfortable environment. Educational program often offered. Current meals sites include Nashville, IN, Seymour, IN, Crothersville, IN, North Vernon, IN and Columbus, IN.
First Steps: Provides case management, assessment, advocacy, and resources to assist families with a child under age 3 diagnosed with a developmental disability
Caregiver Support: Provides support and education to families and caregivers of individuals dealing with dementia
Housing Partnerships: Provides safe and affrodable housing to low-income, seniors and disabled individuals

Description

The Community Health Worker (CHW) serves as an integral member of an individual's support and care team. Using an evidence-based, person-centered approach, the CHW provides goal setting, education, advocacy, care coordination, and practical support to individuals experiencing care transitions, chronic health conditions, disabilities, or barriers related to Social Determinants of Health (SDOH). The CHW works directly with individuals, their circles of support, healthcare providers, and community partners to improve health outcomes, independence, and self-sufficiency. Services are provided in homes, community settings, by telephone, or at designated program locations and are documented in required data systems.

Position Requirements

High school diploma or equivalent required. Community Health Worker certification is required, or the employee must complete approved CHW training and obtain certification within six months of assignment to the role. Two years of relevant experience in community health, healthcare, care management, human services, a community-based organization, or a Medicaid/Medicare environment is preferred. An associate or bachelor's degree in human services, psychology, nursing, public health, social work, or a related field is preferred. Experience working with older adults, individuals with disabilities, vulnerable populations, healthcare providers, state agencies, or Medicaid/Medicare processes is preferred. Must complete all annual and program-specific training requirements.
