Formación

• Bachelor's degree preferred; CHW certification within two years.
• 2+ years in social service delivery with the elderly and families.
• Knowledge of supportive services and community resources.

Responsabilidades

• Document resident interactions, services, and program activities accurately.
• Complete forms, assessments, consents, and documentation for participants.
• Perform resident assessments and care plans to identify needs and goals.
• Coordinate referrals and follow up to ensure access to services.
• Monitor resident care plans and update documentation quarterly.
• Prepare HUD reports and support grant compliance.
• Maintain compliant resident files and program records.
