Job summary

The Developmental Disabilities Administration within the State of Washington is hiring a Health-Related Social Needs Referral and Authorization Case Manager for the Everett office. The role involves HRSN assessments, care planning, and coordinating services with the Health Care Authority to support medically tailored meals, housing, and other supports.

Hybrid/telework options are available with a standard Mon-Fri schedule; travel within the region for client assessments is required.

Qualifications

• Bachelor's degree in social sciences, social services, human services, behavioral sciences or an allied field and two years of experience providing social services to people with intellectual or developmental disabilities.
• Or six years of experience in providing social services to people with intellectual or developmental disabilities.
• Or Satisfactory completion of 12 months as a Case Resource Manager Trainee.
• Or Equivalent experience/education

Responsibilities

• Conduct functional and financial HRSN assessments using CARE Web to determine level of care and eligibility for the HRSN.
• The HRSN assessment will be completed meeting the quality standards set forth in the Long-Term Care Manual.
• Complete initial client assessments and person-centered service plans in CARE within established timelines as required for caseload and according to WAC and send appropriate notices to client and representative.
• Identify support needs of clients of DDA and plan for and authorize payment for appropriate services.
• Obtain translation and interpretation services for clients and families when needed or requested.
• Based on assessed needs, refer individuals to appropriate DDA services, other department services, and community programs/resources.
• Identify clients or families in crisis and make emergent referrals to appropriate authorities, department/administration services, state plan services, or local community resources
