State of Washington in Everett is seeking a Health-Related Social Needs Referral and Authorization Case Manager to support HRSN services for Apple Health clients and coordinate CARE Web assessments. The role may be in-office, telework, or hybrid, with travel within the region for assessments and meetings.

The position requires a social science degree or equivalent experience, strong confidentiality, and cultural sensitivity.

Qualifications

• Bachelor’s degree in social sciences or related field with 2 years of DD social services experience.
• Or six years of social services experience with developmental disabilities.
• Or 12 months as Case Resource Manager Trainee.
• Graduate training may substitute for one year of experience.

Responsabilités

• Conduct functional and financial HRSN assessments to determine care level and eligibility.
• Complete CARE-based assessments and service plans within timelines.
• Identify client support needs and plan payments for services.
• Obtain translation/interpretation services as needed.
• Refer individuals to DDA services and community programs.
• Make emergent referrals for crises to authorities or local resources.
