State of Washington in Everett is seeking a Health-Related Social Needs Referral and Authorization Case Manager to support HRSN services for Apple Health clients and coordinate CARE Web assessments. The role may be in-office, telework, or hybrid, with travel within the region for assessments and meetings.

If you are devoted to serving those with developmental disabilities, consider this exciting opportunity. The Developmental Disabilities Administration is hiring a Health-Related Social Needs Referral and Authorization Case Manager to work in our Everett office.
This project position, expected to last through 30 June 2028, is working closely with the Health Care Authority (HCA) to administer the array of innovative new Health-Related Social Needs (HRSN) services and supports to include medically tailored meals, food and vegetable prescriptions, pantry restocking, air conditioners and installation for high-risk clients with access to cooling, pest eradication services, displacement housing funding, hoarding remediation, and caregiver respite services. The scope of clients HRSN services are available to include all Apple Health eligible clients when deemed medically appropriate using clinical and other health-related social needs.
This position is located in the Everett office, and you have the option to work in office, telework at your home, or we also support a hybrid work model of office and telework. The standard work schedule is Monday – Friday, 8am – 5pm. You may have the option of a modified work schedule after successfully completing your 6-month probationary period. You will be required to attend training outside of your local office. Travel within the region to conduct in-person assessments and meetings with clients and community partners will be required.
Snohomish County employees are eligible to receive an Orca Card for transportation needs.
This position is covered by a collective bargaining agreement.
A Bachelor's degree in social sciences, social services, human services, behavioral sciences or an allied field and two years of experience providing social services to people with intellectual or developmental disabilities.
Or six years of experience in providing social services to people with intellectual or developmental disabilities.
Or Satisfactory completion of 12 months as a Case Resource Manager Trainee.
Or Equivalent experience/education
Graduate training in the areas listed above will substitute, year for year, for one year of the experience providing social services to people with developmental disabilities.
