Job summary

State of Washington Dept. of Social and Health Services is seeking a Health-Related Social Needs Referral and Authorization Case Manager for the Developmental Disabilities Administration in Region 1.

The role involves conducting CARE Web assessments, coordinating person-centered plans, and linking clients to DDCS services and community resources. Occasional travel within the region is required, with a hybrid work schedule after an initial office-based start.

Qualifications

• Bachelor's degree in social sciences, social services, human services or an allied field.
• Two years of experience providing social services to people with intellectual or developmental disabilities, or six years of relevant experience.
• Satisfactory completion of 12 months as a Case Resource Manager Trainee; equivalent education/experience acceptable.
• Graduate training can substitute for one year of experience.

Responsibilities

• Conduct functional and financial HRSN assessments using CARE Web to determine level of care and eligibility.
• Complete initial client assessments and person-centered service plans within established timelines.
• Identify client support needs and plan/authorize payment for appropriate services.
• Arrange translation/interpretation services as needed.
• Refer clients to appropriate DDCS services, other departments, and community programs.
