Job summary

State of Washington Dept. of Social and Health Services (DSHS), Developmental Disabilities Administration (DDA) in Lynnwood, is hiring multiple Case Resource Managers.

You will provide specialized case and resource management to individuals and families eligible for DDA services, with options for in-office, telework, or hybrid work arrangements. The role involves home visits, goal planning, assessments, and coordinating services across agencies.

Qualifications

• Bachelor's degree or six years of relevant social services experience in developmental disabilities.
• Experience providing social services to people with intellectual or developmental disabilities.

Responsibilities

• Identify support needs of clients and plan for and authorize payment for services.
• Schedule and conduct face-to-face home visits with clients and families.
• Facilitate person-centered goal-planning discussions with clients and families.
• Complete and revise client assessments and service plans within timelines.
• Identify clients in crisis and make referrals to authorities or community resources.
• Determine financial and functional eligibility for programs.
• Review and monitor client plans and progress.
• Establish and maintain complete case files and records.
• Represent DDA in community and agency meetings.
