Community Reach Center is seeking a Housing Coordinator to support homeless outreach, document progress toward treatment goals, and provide one-on-one housing support. The role focuses on creating safe environments and implementing housing plans within the HUD project division.

Qualifications

• Bachelor’s degree in psychology, social work, or related field required.
• HQS certification required.
• 1+ years of experience in a similar role or training.
• Valid driver’s license and good driving record.

Responsibilities

• Participates in homeless outreach and HMIS compliance.
• Provide trauma-informed interventions that empower clients in recovery.
• Participates in crisis management and de-escalation per state standards.
• Collaborate with clients to identify strengths and build treatment plans.
• Develop and facilitate therapeutic housing services and assist with case management.
• Report maintenance requests and conduct HQS inspections.
• Coordinate and oversee the State voucher program.
