Job summary

MHA (Mental Health Association) is seeking a Case Manager to provide trauma-informed, person-centered support for individuals transitioning from homelessness to studio apartments within a congregate housing site. You will aid with income stabilization, access to medical care, and employment goals while ensuring accurate documentation.

The role requires bilingual English/Spanish candidates, a valid driver's license, and at least six months of relevant experience.

Qualifications

• Bilingual candidates strongly encouraged (English/Spanish).
• Valid driver's license and reliable transportation.
• Ability to work independently and as part of a team.
• Strong organizational, communication, and interpersonal skills.
• CPR and First Aid certifications may be required.
• Minimum of six months of relevant experience.

Responsibilities

• Coordinate all documentation required for program entry, including homelessness verification, assessments, eligibility screenings, and IDs.
• Complete initial Adult Comprehensive Assessments, updates, and service plans.
• Collaborate with a multidisciplinary team to develop Individualized Action Plans (IAPs).
• Implement IAP strategies and document progress; revise safety plans as needed.
• Assist participants with transition to stable housing and accessing benefits.
