Job summary

Zumbro Valley Health Center in Minnesota is seeking a Full-time Housing Support Practitioner to provide supportive services and skills training to clients enrolled in a housing program. You will screen potential tenants and collaborate with an integrated team to identify needs and connect clients to CCBHC services.

Responsibilities include building relationships with landlords, advocating for clients, developing independent living skills, and ensuring timely documentation.

Qualifications

• Bachelor's degree in Social Work, Psychology or related human services field.
• 1 year of experience serving individuals with mental illness or substance use disorder.
• Housing experience preferred.
• Must qualify as a Mental Health Practitioner in MN; licensure preferred or provisional allowed.

Responsibilities

• Build and maintain professional relationships with landlords.
• Screen potential clients to ensure housing program qualifications.
• Identify resources related to basic housing needs.
• Advocate and link clients to community resources.
• Provide independent living skills training in group and individual settings.
• Contribute to treatment plan team and coordinate with staff.
• Facilitate skills training for clients individually and in groups.
• Model and coach independent living skills to clients.
• Obtain treatment consultation from Clinical Supervisor.
• Document housing and skills sessions accurately and timely.
