Job summary

Housing is Recovery (HiR) in Elgin, IL seeks a Client Support & Service Coordinator / Intake Specialist. The role ensures clients access eligible services, manages referrals, and documents interactions with coordination across agencies.

Requires a bachelor’s degree in human services or related field, strong communication, and proficiency in Microsoft Office. On-site with remote option after six months; schedule is 40 hours weekly.

Qualifications

• Bachelor’s degree in human services or related field.
• Experience in helping/service field and care coordination desired.
• Strong communication, writing, problem solving, and organizational skills.

Responsibilities

• Serve as primary contact for information or enrollment in the Housing is Recovery program.
• Manage referrals and oversee intake to enroll clients into appropriate services.
• Provide general case management and develop individualized service plans.
• Document client interactions using EchoVantage and HMIS.
