• Prepare and maintain required documentation accurately and timely.
• Coordinate with service providers and community resources for comprehensive support.
• Build rapport with participants, showing empathy and cultural sensitivity.
• Promote engagement through peer and natural supports and monitor progress.
• Ensure safety, accountability, and documentation integrity.

Education

Bachelor’s degree in social science

Associate degree + 2 years human services experience

High School Diploma/GED + 4 years human services experience
