Job summary

Compass Career Solutions, sister company to Dungarvin, seeks a Housing team member to provide case management and life skills development for homeless individuals and families. This in-office role requires daily attendance and community outreach to connect clients with medical, mental health, and other supportive services.

You will document care, develop action plans, and coordinate referrals while supporting clients to increase independence and quality of life.

Qualifications

• Bachelor's Degree in social services or related field; minimum one year housing coordination experience.
• High School Diploma or GED with two years housing coordination experience.
• Experience documenting care and communicating with internal and external teams.

Responsibilities

• Conduct assessments and interviews with clients to determine eligibility.
• Develop and update action plans, case notes, and other documents related to a client’s file.
• Rely on reading, writing, and communication skills to document care and follow support plans.
• Provide ongoing life skills development to assist clients in improving independence and quality of life.
• Provide specialized interventions when clients are at risk of eviction.
• Connect clients to medical, mental health, and other supportive counseling services.
• Establish a network of community services and conduct referrals.
• Guide clients through SSI/SSDI/Medicaid/Medicare/SNAP applications and re-determinations.
• Occasionally conduct visits in the client’s home.
• Report outcomes and provide data updates as directed.
