Qualifications

• Bachelor’s degree in a related field and one year of behavioral health case management or community support experience.
• Proficient technology skills for EMR, telehealth, and mobile devices.
• Strong interpersonal and teamwork skills; reliable transportation.

Responsibilities

• Carry a caseload of 25-30 clients and conduct weekly visits.
• Coordinate care and facilitate referrals to services.
• Provide ongoing case management and skills training per treatment plan.
• Gather information from client/family to ensure appropriate care.
• Recognize need for crisis intervention and notify supervisor as needed.
• Document all service contacts within 24 hours.
• Participate in weekly team meetings and meet productivity targets.
• Perform duties assigned by team lead/program manager/director.
