Job summary

Claratel Behavioral Health is seeking a Project HEAL Case Manager to support individuals transitioning from hospital and acute care into transitional housing. Qualifications

• High school diploma or GED is required.
• At least one year of experience in a behavioral health setting providing case management/community support services.
• Basic computer and typing skills required.
• Care coordination skills to connect individuals with resources.
• Transportation to and from work is required.

Responsibilities

• Coordinate discharge planning with hospitals and medical/home health services.
• Plan transitions from facility-based care to residential settings.
• Link clients to outpatient services including counseling, psychiatry, and substance use treatment.
• Coordinate care with residential, medical, behavioral health, and community providers.
• Assist with appointment scheduling, transportation planning, and benefits linkage.
• Monitor engagement in treatment and supportive services after housing placement.
• Participate in multidisciplinary case coordination with Project HEAL partners.
• Maintain documentation per agency policies and regulations.
• Enter data into HMIS / ClientTrack system.
• Perform other duties as assigned by Program Manager/Director.
