Job summary

St. Francis House in Boston seeks a Housing Stabilization Case Manager to support residents transitioning from homelessness to permanent housing, using a Housing First, trauma-informed approach.

You will manage a caseload of about 15 residents, build relationships with partners, develop service plans addressing housing, income, and behavioral health, and document progress in resident records.

Qualifications

• Bachelor’s degree in Social Work or related field with 3+ years of experience preferred.
• In lieu of Bachelor’s degree, High School diploma or GED and 10+ years’ experience will be considered.
• Experience with direct service or case management with vulnerable populations, homeless population preferred.

Responsibilities

• Welcome new residents and support a smooth transition to stable, supportive housing.
• Manage a caseload of approximately 15 residents, with about 30% requiring high-level support.
• Build proactive relationships with residents to ensure connections to services.
• Develop service plans focused on housing, income, and behavioral health stability.
• Coordinate with landlords, providers, and internal departments to address needs.
