Job summary

FACETS is seeking a mission-driven Diversion Specialist to support individuals facing homelessness by identifying safe, immediate alternatives to shelter. The role uses a housing-focused, trauma-informed approach and involves assessments, problem-solving discussions, and referrals to community resources.

The ideal candidate has homelessness services experience and HMIS familiarity, plus strong organizational and communication skills, able to work in a fast-paced, community-based setting.

Qualifications

• Bachelor’s degree in human services or related field required.
• Two years of professional social work/case management with homeless populations or in emergency shelters.
• Bilingual English/Spanish preferred.
• Ability to work with individuals in crisis and coordinate supports.
• Knowledge of local homelessness services and housing resources.

Responsibilities

• Conduct diversion assessments and identify safe alternative housing options.
• Coordinate short-term supports and referrals to community resources.
• Enter and maintain data in HMIS with accuracy and timeliness.
• Maintain client records with confidentiality and compliance.
• Collaborate with shelter intake, outreach and case management teams.
