Qualifications

• Bachelor’s degree in a human services field.
• Master’s degree preferred.
• At least two years of professional experience in mediation, conflict resolution, case management, assessment, counseling, and crisis intervention with homeless or at‑risk populations.
• Community organizing experience is a plus.
• Ability to use client‑centered or motivational interviewing approaches.

Responsibilities

• Participate as a member of the Central Front Door CE unit, attending meetings and updating information according to the established process (within HMIS).
• Meet with the client within 24 hours of assignment.
• Complete comprehensive intake assessment to determine housing options and short-term needs.
• Assess options to avoid shelter by identifying viable short-term housing options.
• Coordinate referrals to housing location services and other case management supports as needed.
• Submit housing subsidy requests and coordinate with landlords or property managers as required.
• Maintain HMIS data and collaborate with DSS and other agencies to meet program goals.
• Provide linkage, follow-up, facilitation and transportation to clients as needed.
• Prepare proposals, reports, and statistics for submission as required.
• Perform other duties as assigned.
