Job summary

The People Concern is seeking a Time Limited Subsidy Case Manager to join the Permanent Housing team at The Nest, LA. You will travel across the greater Los Angeles area to help clients obtain and sustain permanent housing, linking them to employment and services.

You will implement housing stabilization plans, coordinate with landlords and partners, and document care using HIPAA-compliant methods to support client outcomes.

Qualifications

• Valid CA Driver's license with reliable vehicle, auto insurance
• Willing to transport clients in personal vehicle
• Bachelor's degree preferred; or 2+ years case management experience with homelessness or related work
• Demonstrated knowledge of case management
• Ability to work with Harm Reduction model and clients with multiple barriers
• Proficient with basic computer tools (Word, Outlook, Excel)
• CPR/First Aid certification required/maintainable
• Detail oriented with strong time management, organizational, and communication skills

Responsibilities

• Work with a diverse client caseload of up to 20 clients facing homelessness and trauma
• Apply the Whatever It Takes model emphasizing client choice, harm reduction, and trauma-informed care
• Maintain up-to-date biopsychosocial assessments and develop individualized case plans
• Collaborate with clients to create and adjust goal plans for housing stability and well-being
• Maintain accurate client files and ensure data in agency records
• Document contacts in PIP format and in line with Housing Department expectations
• Coordinate with HSH and partners to connect clients with primary health care and insurance
• Maintain client confidentiality per HIPAA and guidelines
• Arrange transportation and appointments for health care, recovery services, and community resources
• Develop rapport to promote independence and health improvement
• Assist with housing applications for Permanent Supportive Housing subsidies
• Participate in staff meetings/trainings and collaborate with BC on documentation
• Perform other duties as assigned
