Job summary

The People Concern seeks a Temporary Case Manager at Kensington Campus in Lancaster, CA. You will support Clients’ housing stability and self-sufficiency using evidence-based practices like harm reduction and housing first, coordinating services for recently housed Clients and facilitating independent living skills.

You will manage a caseload, collaborate with LA County agencies, and advocate for Clients' access to benefits and resources, while maintaining HIPAA-compliant records and

Qualifications

• Bachelor’s degree preferred; or a minimum two years’ experience with homeless individuals.
• Valid CA Driver’s license, reliable car, auto insurance, and acceptable driving record.
• Willing and able to transport Clients in your personal vehicle.
• Able to work in a high tolerance Harm Reduction model with Clients experiencing multiple barriers.

Responsibilities

• Maintain a caseload of high-acuity Clients for intensive case management and housing retention.
• Develop relationships with Clients to support independence and improved health.
• Coordinate with DMH/DHS programs to enhance treatment team collaboration.
• Engage Clients with a harm-reduction approach and trauma-informed care.
• Assist in screening, intake, orientation to program policies and goals.
• Complete biopsychosocial assessments and guide individualized treatment planning.
• Develop quarterly goals to improve quality of life and health outcomes.
• Write and maintain progress notes in electronic databases; ensure chart accuracy.
• Maintain confidentiality of Client information per HIPAA and agency guidelines.
• Assist Clients in applying for and obtaining benefits; coordinate appointments and transportation.
• Directly transport Clients to providers in the greater Los Angeles area to reduce barriers.
• Teach life skills such as budgeting, housekeeping, and scheduling to support housing retention.
• Communicate with property managers and third-party partners; participate in team meetings.
• Adapt workload as directed by supervisor to meet funder expectations.
