Qualifications

Responsibilities

• Provide high quality case management to clients within the Intensive Case Management Services program.
• Assist clients in transitioning from homelessness to tenant living, including housing subsidies and documentation.
• Maintain caseload and conduct regular face-to-face sessions with scattered site participants.
• Prepare housing-related forms and coordinate with housing authorities for voucher approvals and recertifications.
• Conduct on-site visits, home visits, and ongoing case management to monitor progress and arrange referrals.
