Job summary

Beacon Hill Staffing Group, LLC is seeking a Clinical Case Manager in Boston to provide case management and supportive counseling to individuals experiencing homelessness and co-occurring mental health and/or substance use concerns. The position is full-time, with a schedule of Monday–Friday, 7:00 AM–3:00 PM EST, and pay up to $26.50 per hour as a contract-to-hire role.

Qualifications

• Bachelor's degree in a human services-related field; relevant experience may be considered in lieu of a degree.
• 3+ years of experience in case management with individuals experiencing substance use or mental health concerns.
• Knowledge of Harm Reduction, Motivational Interviewing, Relapse Prevention, Trauma-Informed Care, and related models.
• Experience with crisis intervention and de-escalation and maintaining professional boundaries.
• Strong written and verbal communication skills and ability to document progress notes and service plans.
• Strong computer and electronic recordkeeping skills; LADC or CADC credential is a plus.

Responsibilities

• Manage an assigned caseload and connect participants with services and community resources.
• Provide supportive counseling using evidence-based approaches including Motivational Interviewing, Harm Reduction, Relapse Prevention, and Trauma-Informed Care.
• Complete biopsychosocial assessments, individualized service plans, discharge plans, progress notes, and other required documentation.
• Work collaboratively with participants toward permanent housing, increased income and benefits, greater stability, and improved self-determination.
• Respond to crisis situations and provide appropriate intervention and de-escalation while helping maintain a safe environment.
• Coordinate community-based referrals and provide appointment accompaniment when needed to support continuity of care and independent living.
• Collaborate with clinical and operational teams while participating in case conferences, program meetings, and other required meetings.
