Job summary

Union Station Homeless Services seeks a Care Coordinator to provide case management through the PATH program, linking participants to housing, medical, mental health, and support services to promote housing retention and wellness.

This role requires coordinating enrollment, developing Housing and Services Plans, and conducting monthly home visits as needed to support clients’ goals and financial independence.

Qualifications

• Bachelor's degree in Social Work or related field is required.
• 2+ years of experience in a related human services field preferred.
• Experience with housing resources and supportive services is strongly preferred.

Responsibilities

• Manages a caseload of 20–25 households enrolled in PATH.
• Enrolls referrals and creates Housing and Services Plans with participants.
• Locates housing units, verifies ownership, inspects units, and submits rental packets.
