Job summary

Health Care For the Homeless Inc. is seeking a licensed social worker to deliver therapy and intensive case management for adults experiencing homelessness.

You will provide trauma-informed assessments, individual and group therapy, and collaborate with an interdisciplinary team to support housing stability. The role requires a Master’s in Social Work and an LMSW or LCSW-CP license with Maryland eligibility.

Qualifications

• Master's Degree in Social Work from an approved school.
• Current license as an LMSW or LCSW-CP in Maryland.
• 1 year experience in therapy for individuals with substance use and mental health disorders preferred.
• 1 year experience in case management for housing stability preferred.

Responsibilities

• Provide behavioral health assessments, individual and group therapy, and intensive case management to support housing stability.
• Perform home and community visits and escorts as per client plan of care.
• Collaborate with an interdisciplinary care team and external agencies.
• Develop integrated treatment plans with SMART goals.
• Coordinate care with landlords and agencies to facilitate housing outcomes.
• Complete documentation in electronic health records within required timeframes.
