Job summary

The Site Manager position at Covenant Case Management Services oversees non-clinical, person-centered Tenant Case Management services for Stanly County residents. You will support housing stability, life skills, and access to community resources, following the Vaya Health program requirements.

Responsibilities include maintaining member contact, coordinating with behavioral health and housing partners, developing plans, and documenting interactions while upholding HIPAA and Medicaid standards.

Qualifications

• High School diploma or equivalent is required.
• Valid driver’s license with reliable transportation.
• Ability to pass background check and required credentialing.
• Strong communication, documentation, and interpersonal skills.

Responsibilities

• Maintain scheduled contact with assigned members to support housing stability and independent living.
• Link members to behavioral health, medical, community resources as needed.
• Assist in creating and updating Person-Centered Plans, Crisis Plans, or Housing Stabilization Plans.
• Provide education, engagement, and support to members and families/caregivers using non-clinical approaches.
• Coordinate services with local vendors and housing program staff.
