Covenant Case Management Services is seeking a Site Manager to provide non-clinical, person-centered support to tenants in Stanly County. You will help maintain housing stability, connect clients with behavioral health and community services, and support life skills and resiliency goals under the Tenant Case Management program.

The role requires strong communication, documentation, and interpersonal skills, with travel to housing sites and strict adherence to HIPAA and Medicaid regulations.

Qualifications

  • High School diploma or equivalent.
  • Valid driver’s license, reliable transportation and ability to travel to work site.
  • Ability to pass background check and required credentialing.

Responsibilities

  • Maintain scheduled contact with assigned members to support housing stability, independent living skills, and crisis prevention.
  • Assist members with developing and achieving whole health and resiliency goals.
  • Link members to behavioral health, medical, community, and natural support resources as needed.
  • Participate in the creation and updates of Person-Centered Plans, Crisis Plans, or Housing Stabilization Plans.
  • Provide education, engagement, and support to members and their families/caregivers using non-clinical approaches.
  • Document all service contacts and interventions using required logs and monthly service notes.
  • Assist clients with the completion of annual recertification documentation.
  • Coordinate services with local vendors.

Site Manager – Tenant Case Management Program
Reports To: Clinical Professional Development Director
Location: Stanly County, On-Site
Job Type: Full-Time
Schedule: 2:30pm - 8:30pm, Monday-Sunday

Position Summary

The Site Manager is responsible for providing non-clinical, person-centered support services to
individuals receiving Tenant Case Management through Covenant Case Management Services.
This position assists members in maintaining housing stability, accessing behavioral health and
community services, building life skills, and supporting whole health and resiliency goals in
accordance with the Vaya Health Tenant Case Management program requirements.

Key Responsibilities

Member Support & Service Delivery

  • Maintain scheduled contact with assigned members to support housing stability,
    independent living skills, and crisis prevention.
  • Assist members with developing and achieving whole health and resiliency goals.
  • Link members to behavioral health, medical, community, and natural support resources
    as needed.
  • Participate in the creation and updates of Person-Centered Plans, Crisis Plans, or
    Housing Stabilization Plans for members not linked to a behavioral health provider.
  • Provide education, engagement, and support to members and their families/caregivers
    using non-clinical approaches.
  • Maintain a working knowledge of the Eviction Prevention Protocol and support
    intervention steps as needed.

Documentation & Reporting

  • Document all service contacts and interventions using required logs and monthly service
    notes in accordance with program standards.
  • Assist clients with the completion of their annual recertification documentation.
  • Submit accurate and timely documentation, signatures, and required reports.
  • Maintain applicant/waitlist tracking as assigned and support Residential Selection
    Committee processes.
  • Maintain confidentiality and comply with HIPAA and Medicaid regulations.
  • Assist clients with required work order and maintenance forms.
  • Coordinate services with local vendors.

Collaboration & Communication

  • Work closely with Vaya Health Residential Housing Specialists, ARC of NC Property
    Management staff, and internal Covenant Case Management team members.
  • Participate in interviews for housing applicants as requested.
  • Notify supervisors of member concerns, safety risks, or eviction risks promptly.
  • Represent the agency in a professional and ethical manner in community settings.

Qualifications

Required:

  • High School diploma or equivalent
  • Valid driver’s license, reliable transportation, and ability to travel to work site
  • Ability to pass background check and required credentialing
  • Strong communication, documentation, and interpersonal skills

Preferred:

  • Experience working with MH/SU/IDD/TBI populations
  • Experience in case management, supportive housing, or human services
  • Associate or Bachelor’s degree in Human Services, Psychology, Social Work, or related
    field

Work Environment & Physical Requirements

  • Field-based position requiring travel to housing sites
  • Ability to sit, stand, walk, and lift up to 25 lbs occasionally
  • Must maintain professional boundaries and safety practices
