Job summary

The Covenant Case Management Services site in Stanly County is seeking a Site Manager for the Tenant Case Management Program to provide non-clinical, person-centered support to members housing-stability goals. This role collaborates with health and community partners to connect residents with services and resources.

Responsibilities include maintaining regular contact, developing Plans, coordinating with providers, and documenting all interactions in compliance with HIPAA and program standards.

Qualifications

• Must have high school diploma or equivalent.
• Valid driver's license and ability to travel to work sites.
• Ability to pass background check and credentialing.
• Strong communication, documentation, and interpersonal skills.

Responsibilities

• Maintain scheduled contact with assigned members to support housing stability and independent living skills.
• Assist members with developing and achieving whole health and resiliency goals.
• Link members to behavioral health, medical, community, and natural support resources as needed.
• Participate in creation and updates of Plans (Person-Centered, Crisis, Housing Stabilization).
• Provide education, engagement and support to members and families/caregivers using non-clinical approaches.
• Document all service contacts and interventions per program standards.
