The Covenant Case Management Services site in Stanly County is seeking a Site Manager for the Tenant Case Management Program to provide non-clinical, person-centered support to members housing-stability goals. This role collaborates with health and community partners to connect residents with services and resources.
Responsibilities include maintaining regular contact, developing Plans, coordinating with providers, and documenting all interactions in compliance with HIPAA and program standards.

Schedule: 2:30pm - 8:30pm, Monday-Sunday
The Site Manager is responsible for providing non-clinical, person-centered support services to individuals receiving Tenant Case Management through Covenant Case Management Services. This position assists members in maintaining housing stability, accessing behavioral health and community services, building life skills, and supporting whole health and resiliency goals in accordance with the Vaya Health Tenant Case Management program requirements.
