Job summary

Easterseals PORT Health in Burlington, NC, seeks a Housing Specialist to join the Assertive Community Treatment team serving Alamance County. You will help individuals with severe mental illness secure and maintain safe housing while developing life skills.

This role emphasizes tenancy support, care coordination, and rehabilitation services, with responsibilities including case management, treatment planning, and collaboration with housing authorities and landlords.

Qualifications

• Case management for individuals with severe mental illness.
• Knowledge of housing resources and tenancy supports.
• Recovery‑oriented, person‑centered care approach.

Responsibilities

• Provide case management for an assigned group with ITT coordination.
• Develop, implement, review, and revise individualized treatment goals and service plans.
• Deliver rehabilitative services including housing/tenancy supports, money management, ADLs, health and recovery.
• Build and maintain relationships with housing authorities, landlords, and community programs.
• Coordinate communication with guardians, care coordinators, and housing partners.
• Provide supportive therapy and symptom management; participate in assessments and on‑call rotation.
• Document services in the electronic health record and participate in team meetings.
