Job summary

Easterseals PORT Health is seeking a compassionate Housing Specialist to join our ACT team in Alamance County, NC. You will help individuals with severe mental illness secure and sustain safe housing, while building skills for long‑term community success.

As part of a multidisciplinary team, you will coordinate care, deliver rehabilitation services, and advocate for client rights. This role follows a Monday–Friday schedule with some on‑call responsibilities and travel within Burlington and

Qualifications

• Experience supporting individuals with severe mental illness is required.
• Knowledge of housing resources and tenancy supports is preferred.
• Bachelor’s degree in a human services field is required.

Responsibilities

• Provide case management and coordinate care with the ACT team.
• Implement individualized treatment goals and service plans.
• Deliver housing and tenancy‑sustaining supports, money management, and ADL assistance.
• Engage housing stakeholders and coordinate with guardians, landlords, and agencies.
• Participate in assessments and on‑call rotation to support clients.
• Document progress in the electronic health record and participate in team meetings.
