Job summary

Easterseals PORT Health in Durham, NC is seeking a full-time Registered Nurse (RN) to join our Assertive Community Treatment Team. You’ll provide community-based behavioral health care, including psychotropic medication management, nursing assessments, care coordination, and crisis response in homes, shelters, and community settings.

This role requires an NC RN license, strong communication skills, and willingness to travel across Durham, Person, and Orange Counties.

Qualifications

• Requires active RN license in North Carolina.
• Experience in behavioral or mental health is preferred.
• Comfort working in community-based settings.

Responsibilities

• Administer and monitor psychotropic medications as prescribed.
• Conduct nursing assessments and track symptoms, side effects, and medical issues.
• Document care and observations in the EMR.
• Coordinate care with psychiatrists and other professionals.
• Provide crisis assessment and intervention as part of on-call rotations.
• Deliver nursing care in clients’ homes and community settings.
