Job summary

Easterseals PORT Health in Alamance County seeks a compassionate Housing Specialist/Qualified Professional to support individuals with severe mental illness in securing and maintaining safe housing. You will work within the ACT team to provide housing stability, tenancy support, rehabilitation services, and transition to community living initiatives.

This full-time role requires a Bachelor’s degree in a human services field, QP eligibility, and the ability to coordinate with housing authorities,

Qualifications

• Experience with severe and persistent mental illness populations.
• Knowledge of community housing resources and local housing stakeholders.
• Ability to coordinate care with multidisciplinary teams and outside agencies.

Responsibilities

• Provide case management and coordinate care with ITT.
• Develop and revise individualized treatment goals and service plans.
• Deliver housing/tenancy supports, money management, ADLs, and recovery-oriented services.
• Engage housing authorities, landlords, and service providers to sustain housing.
• Participate in assessments covering psychiatric history, health, social functioning, and housing needs.
• Support on-call coverage and crisis response as part of ACT.
