Job summary

Easterseals PORT Health in Durham, NC seeks an experienced Registered Nurse to join our Assertive Community Treatment (ACT) Team. This full-time role delivers community-based behavioral health care to adults in homes, shelters, and community settings, focusing on stability and independence.

You will administer psychotropic medications, perform comprehensive nursing assessments, coordinate care with psychiatrists and clinicians, and respond to crises on a rotating on-call schedule.

Qualifications

• Active, unrestricted RN license in North Carolina.
• Experience in behavioral health or mental health preferred.
• Valid driver’s license and reliable transportation.
• Strong communication and documentation skills.
• Ability to work in homes, shelters, and community settings.

Responsibilities

• Medication & Clinical Care: Administer and monitor psychotropic medications as prescribed; conduct nursing assessments; document care in the EMR.
• Care Coordination: Partner with team psychiatrist to update medication protocols and care plans; participate in daily ACT Team meetings.
• Crisis Response: Provide crisis assessment and intervention as part of rotating on-call schedule.
• Community-Based Support: Deliver nursing care in clients’ homes and community settings; educate on health management and medication adherence; advocate for client choice.
