Job summary

Project Renewal in New York is seeking a Housing Case Manager to support residents across multiple housing sites in the Bronx, Brooklyn and Manhattan. You will conduct assessments, develop service plans, coordinate health services and connect tenants with community resources.

You will maintain client records, participate in weekly clinical meetings and provide counseling and education on financial matters and employment opportunities to help clients achieve independence.

Qualifications

• Bachelor’s Degree in Social Work or related field required.
• 1–2 years experience with homeless mentally ill adults or similar population preferred.
• Excellent oral, writing and listening skills.
• Proficiency with computers.
• Bilingual or multilingual abilities a plus.

Responsibilities

• Provides intake, psychosocial and substance abuse assessments and develops treatment plans.
• Provides case management to create and monitor individual Service Plans.
• Conducts home visits and coordinates medical, psychiatric and substance abuse services.
• Monitors medication adherence and maintains client case notes in AWARDS/CARES.
• Conducts individual and group counseling and educates tenants about money management and employment options.
• Links clients to community services and participates in weekly clinical meetings.
