Job summary

Bowery Residents' Committee is seeking a dedicated case manager to support chronically homeless individuals through comprehensive assessment, planning, and coordination of housing services. You will meet clients regularly in the field and in the office, ensuring complete and up-to-date clinical files and reports.

Qualifications

• High School Diploma/GED required; bachelor's degree preferred and masters ideal.
• Bilingual Spanish strongly preferred, but not required.
• Excellent communication and writing skills and computer proficiency required.
• Experience in residential treatment and with the MICA and homeless populations.
• Knowledge of HRA and NY/NY application systems a plus.
• CPR training certification or willingness to take training class in CPR.

Responsibilities

• Provide comprehensive case management services to identified chronic homeless individuals.
• Manage all aspects of assigned caseload with regular client meetings in field and office.
• Conduct comprehensive assessments to identify housing barriers and assist with entitlements.
• Develop and implement individualized housing plans and coordinate support services.
• Prepare clients for housing interviews and arrange appointments; participate in case conferences.
• Liaise with clinical staff in BRC or outside programs for coordinated care.
• Provide emergency first aid/CPR or overdose prevention protocol assistance when needed.
