Qualifications

• Bachelor's Degree in Social Work or related field.
• 1-2 years of experience with homeless mentally ill adults or similar populations preferred.
• Excellent oral, writing and listening skills.
• Proficiency in computer use required.

Responsibilities

• Provide case management and counseling by conducting intake, psychosocial and housing assessments.
• Identify entitlements and housing or concrete services to assist clients.
• Develop Independent Living Plans with clients and monitor progress.
• Refer clients to community treatment and support services as needed.
• Counsel clients in wellness, sobriety, employment, and independent living skills.
• Collaborate with on-site providers to ensure aftercare and proper documentation.
