East Williamsburg Men’s Shelter provides emergency housing and comprehensive case management to men ages 55+ with histories of substance use.

The Case Manager will handle a caseload of 25–35 clients, conduct intake and psychosocial assessments, develop Individual Living Plans, document in CARES, and support clients toward permanent

Qualifications

• Conducts intake and psycho-social assessments for clients.
• Provides case management and assists in creating and adhering to Individual Living Plans.
• Maintains up-to-date client documentation per city/state guidelines.

Responsibilities

• Manage a caseload of 25-35 clients.
• Conduct intake and assessments, including psycho-social evaluations.
• Develop and monitor Individual Living Plans with short- and long-term objectives.
• Input client data into the CARES system.
• Counsel clients through individual and group sessions.
• Liaise with community organizations and housing providers.
• Organize group activities and client education.
• Maintain accurate documentation per guidelines.
• Provide escorts to meetings and appointments.
• Perform other duties as assigned by the Director of Social Services.
